Mercy Heart Health Clinic provides medical and dental services free of charge to all patients who

• Are residents of Rockdale County, Ga.;
• Have no medical or dental insurance; and
• Are 200 percent below the poverty level.

All medical and dental services are provided by a licensed professional. Physician’s Assistants and Nurse Practitioners are supervised by a licensed M.D.

Mercy Heart provides basic medical care for those suffering from the following chronic diseases:

• Diabetes
• Hypertension
• Chronic Heart Failure (CHF)
• COPD

Mercy Heart also provides basic dental care, including

• Cleanings
• Extractions
• Fillings

Mercy Heart Clinic will provide care that best meets the needs of the patient in the above listed areas within the limitations of the practice. The healthcare provider has the right to restrict further care upon evaluation of the patient.
